2000 Ford Explorer EGR? Vac line?

I have a 2000 Ford Explorer 5.0 engine. I have a plastic green line coming off of the EGR valve and two (one red and one white) plastic line coming off of the canister next to the EGR valve. All three of these lines were broken when trying to replace the EGR valve. Trouble code 1401 has came up again stating there is an insufficent flow in the EGR system. HOWEVER, the egr valve was replaced like 20 thousand miles ago. The troublecode was erased but came back on and eventually went out. Now it's back on. I tried reconnecting the plastic lines using rubber boots and the light never came back on until today. I've also noticed whenever going up hill or holding the accelerator steady at a certain point the engine stumbles a bit. Could this be from the plastic lines or possible needing EGR valve?
